Services for those Experiencing a Loss of Independence

We offer a wide range of services for people experiencing a loss of autonomy due to aging and for people with physical disabilities, as well as for their families and caregivers.

The services we offer are based on an individualized needs assessment and also take into account the person’s abilities and resources, as well as the resources available within their environment, and include:

  • An assessment of the individual’s overall health, the degree of loss of independence, the disability and resources
  • Referral to the appropriate clinical and professional services and home support services to compensate for temporary or permanent limitations 
  • Nursing care
  • Psychosocial services 
  • Rehabilitation services (occupational therapy and physical therapy)
  • Nutrition services 
  • Home aid services for assistance with activities of daily living
  • Medical services 
  • Palliative care 
  • Various programs (falls prevention, home modifications, etc.)
  • Respite and support services for informal caregivers 

Services can either be provided at the CLSC or at home, as long as the client meets the eligibility requirements for home support services. Depending on the individual’s condition, they can be referred to:

  • A day centre 
  • A day hospital 
  • A rehabilitation unit 
  • A temporary residential unit  
  • A permanent residential unit 
  • A community organization 
  • A social economy enterprise
