How to access your medical record?
Send your duly completed documents to the Medical Records Department by mail, fax, email or in person.
Your medical record or that of your child under the age of 14
Fill out the Authorization to release information contained in the medical record form and include the following information:
- Your first and last name (or that of your child under the age of 14)
- Your date of birth (or that of your child under the age of 14)
- The first and last name of your mother and your father (or the full name of your child under the age of 14)
- Your full address and a phone number where you can be reached
- Indicate which documents you are requesting, along with their date
- Don’t forget to mention whom the documents are intended for. If they are intended for a healthcare professional, specify their name and the full address of their place of practice.
A deceased person’s medical record
Fill out the Authorization to release information contained in the medical record form and include the following information:
Concerning the deceased:
- Full name
- Date of birth
- Date of death
Information about yourself:
- Your full name
- Your date of birth
- Your full address and a phone number where you can be reached
- Provide specific reasons for your request
- Provide a proof* of relationship
- Specify the information you need**
- Indicate whom your request is intended for. If your request is intended for a healthcare professional, specify their name and the full address of their place of practice

*In the case of the search for a hereditary or genetic disease, please specify the name of the disease (a medical note mentioning the specific disease may be required).
Processing times and fees
Please note that requests are processed within a prescribed time limit; no information shall be sent by email.
As provided for under section 26 of the Act respecting health services and social services, the institution shall give the user access to his record as soon as possible.
It should be noted that the first 20 photocopies are provided free of charge. Also note that you will be billed based on the number of exceeding copies or documents requested.
