An innovative rehabilitation program for seniors!

Rehabilitation services for seniors discharged after a hospital stay are essential to helping them regain their functional independence. However, once they are back home, access to outpatient physiotherapy continues to be limited in spite of the need.To optimize the physical abilities acquired during rehabilitation, a new post-acute home care program (SPAD) was launched, providing in-home physical rehabilitation therapy for a maximum of 8 weeks. The program ran for two years and has been reviewed to explore the effects on the users’ health and functional independence.
This webinar will present the results of the review as well as areas for improvement.
